Case managers work one-on-one with shelter guests to assess their barriers to living independently. Together they create an individualized plan with specific goals to overcome those barriers. These Individualized Service Plans are created in cooperation with guests and tailored to each individual's needs.
The goals of our shelter case management programs are to ensure that guests leave with:
- A family sustaining income
- Stable mental health, including sustained sobriety
- Sustainable permanent housing
Our shelter case management programs are designed to serve the following populations:
Individuals with few barriers who might only need assistance finding stable housing or income.
Individuals with multiple barriers including, access to stable housing and income; life-skills coaching; job readiness training; addiction recovery counseling; and/or mental health counseling.
Individuals with barriers related to physical, medical, or mental health needs that are referred to facilities better equipped to serve them (e.g. senior living, group homes, full-time care facilities).
We tailor our case management services to the specific needs facing our nation's veterans by working closely with the Department of Veterans Affairs in our LEADS for Vets program.
Since 2010, the Guest House has enhanced our traditional case management services with new programming aimed at rapid rehousing and preventing homelessness, including:
A program with the goal of quickly establishing stable housing for newly homeless individuals to avoid extended periods in shelter. Rapid Rehousing provides case management services including housing search assistance. This is a "housing first" program.
Community Outreach Case Management
This program works with individuals and families facing the possibility of losing their housing, to link them with resources to stabilize their housing so as to avoid homelessness. Community Outreach Case Managers provide support through advocacy and referral to resources so clients can build their community of support. The goal of this program is to help prevent people from entering the shelter system.
